What’s included

This service provides a structured assessment process to determine whether ADHD is supported as part of your clinical presentation.

✔ Standardized ADHD rating scales (self + optional observer report)

✔ Detailed clinical interview (50 minutes)

✔ Review of current functioning and developmental history

✔ Screening for co-occurring factors (e.g., anxiety, trauma, mood)

✔ Clinical integration of findings

✔ Written summary report with clinical recommendations

✔ Optional cover letter / collaboration with medical professionals

Fees

Total ADHD Assessment: $400

Includes questionnaires, interview, scoring, and report.

Deposit required to book — incomplete forms will result in rescheduling.

Limitations

Structured clinical ADHD assessment by a Registered Clinical Social Worker (RCSW). Written report provided for treatment planning and to support workplace or academic accommodation requests where accepted. Not a comprehensive psychoeducational or neuropsychological assessment. Some institutions require specific types of documentation—confirm this assessment meets your requirements if needed.
